Background Liver cancer remains one of the most fatal malignancies worldwide, with substantial heterogeneity in etiology across regions. We quantified the global economic burden attributable to liver cancer in 2023 across 183 countries using a value-of-lost-welfare (VLW) framework. We evaluated total liver cancer and major etiologic subtypes, reporting absolute VLW, per-capita losses, and VLW as a percentage of GDP (VLW/GDP).Methods Country-level liver cancer disability-adjusted life years (DALYs) for 2023 were extracted from the Global Burden of Disease (GBD) Study ( IDDF2026-ABS-0535 Figure 1. Value of lost welfare (VLW)/gross domestic product (GDP) in 2023 by Global Burden of Disease super-region for Liver Cancer and its Subtype). GDP (PPP) data were obtained from World Bank indicators. VLW was estimated by multiplying DALYs by a uniform monetary valuation per DALY (USD 14,403.29), consistent with established welfare-loss methodology. We computed VLW, VLW/GDP (%), and performed subtype-specific analyses: hepatitis B–related, hepatitis C–related, alcohol-related, NASH-related, and other-cause liver cancer, as well as hepatoblastoma. Aggregated estimates were generated for seven GBD super-regions.Results Globally, total liver cancer caused 8.43 million DALYs in 2023, corresponding to a VLW of USD 121.3 billion, equal to 0.076% of global GDP. The highest absolute losses were observed in China (USD 32.0 billion), India (USD 12.4 billion), Japan (USD 9.9 billion), United States (USD 7.7 billion), and Indonesia (USD 3.8 billion). Across subtypes: HBV-related liver cancer caused the largest global losses (3.32 million DALYs; USD 47.8 billion), HCV-related liver cancer accounted for 1.99-million DALYs (USD 28.6 billion), Alcohol-related liver cancer contributed 1.46 million DALYs (USD-21.1 billion), NASH-related liver cancer accounted for 0.86 million DALYs (USD 12.4 billion), Other-cause liver cancer contributed 0.77 million DALYs (USD 11.1 billion), Hepatoblastoma accounted for 28,000 DALYs (USD 405 million). By super-region, VLW/GDP (%) was highest in Southeast Asia, East Asia & Oceania (0.14%), followed by North Africa & Middle East (0.10%), Central Europe/Eastern Europe/Central Asia (0.08%), and South Asia (0.07%).Conclusions Liver cancer imposes a substantial global welfare loss exceeding USD 120 billion annually, with sharply elevated burdens in East and Southeast Asia, driven primarily by HBV and HCV. Countries with the highest VLW/GDP ratios—including Mongolia, Egypt, Vietnam, and Thailand—represent priority targets for enhanced screening, HBV vaccination, antiviral treatment, and metabolic liver disease prevention strategies.
