Introduction Headaches are a leading reason for neurology referrals, making up 33% of annual outpatient referrals to The Walton Centre. Given the range of potential diagnoses, the Trust, in partnership with Tata Consultancy Services (TCS), developed a chatbot to streamline history-taking, reduce wait times, and improve operational efficiency and patient experience in the headache referral pathway. TCS expertise ensured compliance with regulatory and industry standards. Methods Patients referred on a routine basis for headache completed a questionnaire on common headache disorders via the chatbot, which generated a Patient Summary Report. These are reviewed by clinicians alongside the referral, aiding in identifying red flag symptoms, provide pre-appointment advice where appropriate (e.g. medication overuse), and visualise referral trends. Results In an audit from the pilot trial, 80% of clinicians found the chatbot valuable in adding information to the referral, while 89% of patients indicated willingness to complete it within the referral pathway. Conclusion The novel use of chatbot technology in triaging headache referrals optimises referral pathways, reducing waiting times, with patient acceptance. The platform could expand to offer treatment advice to primary care clinicians or patients or be adapted for other neurological conditions with symptomatic severity but low risk of serious outcomes.hltboey@liverpool.ac.uk
