Acute degeneration of a pedunculated uterine leiomyoma during in vitro fertilisation

A woman in her late 30s with 14 months of unexplained infertility underwent controlled ovarian stimulation (COS) for in vitro fertilisation (IVF). She had one prior live birth. Ovarian reserve testing demonstrated an anti-Müllerian hormone level of 3.5 pmol/L and an antral follicle count of 10. Her body mass index was 32 kg/m². Her gynaecological history was pertinent for two asymptomatic, non-cavity-distorting leiomyomata: an International federation of gynecology and obstetrics (FIGO) type 7 left posterior (pedunculated) measuring 7.6×7.5×5.8 cm and a FIGO type 5 right lateral measuring 5.8×6.5×3.9 cm ( figure 1). Surgical management was deferred before IVF given the absence of clinical symptoms and cavity distortion.
