As I step down as Editor-in-Chief of BMJ Open Sport & Exercise Medicine (BOSEM), I find myself reflecting not only on the journal itself but also on the broader publishing environment in which we all participate. Over the years, I have seen inspiring research, genuine collaboration and remarkable commitment from reviewers. I have also learnt, often uncomfortably, how strongly structural forces shape what is published and what is not.
