Introduction Primary treatment for isolated PCL injuries is typically non-operative. The aim of this study was to compare function and knee laxity after PCL injury following rehabilitation with or without dynamic PCL orthosis.Materials Patients with acute (< 3 weeks) PCL injury were randomized to rehabilitation with or without PCL orthosis. The orthosis was worn continuously for 8 weeks, plus 4 weeks during load-bearing. KOOS function score and Tegner activity score were measured at injury and one year after. Knee laxity was clinically measured with the posterior drawer test (graded 1 to 3). A hop test was performed one year after injury.Results Of 80 included patients, 72% (n=57) had an isolated PCL injury, and 28 % (n=22) had an additional injury to one collateral ligament. Mean Tegner score before injury was 6 and remained unchanged one year after. 68 % (n=55) jumped better on the injured leg. No significant differences were found between the groups for KOOS subscore Sport (mean 83 vs 76, p = 0.63) and Knee-Specific Quality of Life (mean 78 vs 74, p = 0.36) afte one year. 78 % had the same or worse knee laxity from injury to one-year follow-up. 7 % (n=5) of the patients later underwent PCL reconstruction.Conclusion No significant differences were found between the groups in self-reported knee function, activity level, or improvement in knee laxity one year after injury. However, the groups are small, so we cannot rule out that the PCL orthosis may have an effect in selected patients.
