P433 Impact of a simulation based upper gastrointestinal bleeding course on endoscopist confidence and technical competence

Introduction Upper gastrointestinal bleeding (UGIB) remains a highrisk emergency requiring timely and technically proficient endoscopic intervention. This study evaluated the impact of the Sheffield Upper GI Bleeding Course on improving participants’ confidence, technical competence and psychological preparedness when performing acute UGIB endoscopy.Methods Delegates attending the oneday Haemostasis Course at Sheffield Teaching Hospital between November 2021 and September 2024 completed pre and postcourse questionnaires. The course delivered a highfidelity simulated learning environment incorporating casebased teaching and handson training using synthetic and porcine models, supported by consultant faculty and industry partners. Outcomes were assessed using fivepoint Likert scales across domains including practical haemostatic skills (adrenaline injection, clip placement, variceal and nonvariceal therapy, Bentonite haemostatic agent use), operator confidence (mental preparedness, satisfaction, frustration) and psychomotor performance. Pairedsample ttests (SPSS v20) evaluated changes across domains.Results Seventyeight matched participant responses were analysed: gastroenterology trainees (73.1%), consultant gastroenterologists (12.8%), surgical consultants (3.8%), endoscopy nurses (3.8%), nurse endoscopists (2.6%) and nontraining grades (3.8%). Overall, 64% were currently or imminently undertaking an emergency ‘Bleeds Rota’. Baseline experience varied, with 43.6% having performed 1–9 adrenaline infiltrations, 53.8% placing 1–10 clips, and 55.1% using thermal therapy fewer than 10 times. Significant improvements were observed across all assessed domains, with increased confidence, enhanced technical skills and reduced frustration and perceived workload ( table 1).Conclusions The BSG- Sheffield UGIB Haemostasis Course produced measurable gains in participants’ confidence, psychomotor ability and haemostatic technique, while reducing cognitive burden during simulated acute bleeding scenarios.
