A 30-year-old right-handed man from Iraq had several weeks of progressive dull headache, worse on waking and on lying, but without vomiting, visual symptoms or focal neurological deficit. MR scan of brain showed a solitary, well-circumscribed cystic lesion in the left cerebral hemisphere, suggesting hydatid disease. Systemic imaging showed additional cystic lesions in the liver and both lungs, supporting disseminated cystic echinococcosis.
