Early high-energy enteral nutrition in acute pancreatitis: overfeeding and respiratory risk in high-risk patients

I read the GOULASH trial with great interest. This multicentre, double-blind randomised clinical trial compared early high-energy enteral nutrition (30 kcal/kg/day) with a gradually increasing energy strategy (0–30 kcal/kg/day, escalated over 4 days) in acute pancreatitis.1 Following the interim analysis, the trial was stopped early because the high-energy group did not improve the primary outcome (mortality plus severe acute pancreatitis) and might even have increased the risks of organ failure and pain relapse. This study fills an important gap in current acute pancreatitis guidelines regarding energy prescription and provides direct evidence for clinical nutritional support. Despite this important contribution, several aspects of the findings merit further discussion.
