Management of asymptomatic high-grade carotid stenosis has remained a matter of debate as advances in best medical therapy and revascularization techniques have challenged the relevance of earlier trials favoring carotid endarterectomy.1 2 The Carotid Revascularization and Medical Management for Asymptomatic Carotid Stenosis Trial (CREST-2) evaluated whether carotid revascularization—either percutaneous carotid artery stenting or carotid endarterectomy—provides additional benefit beyond contemporary intensive medical therapy alone.3
