A man in his mid-70s presented with a 1-month history of progressively worsening right upper limb ballistic movements and an unsteady gait. This man was known to have diabetes mellitus type 2 and was on insulin treatment. Despite having normal serum blood glucose levels on presentation, the serum glycated haemoglobin (HbA1c) test was abnormally high.
