This presentation explores the development of the 2024 SA Advance Care Directive (ACD) Digital Resource Pack and highlights the role of Local Government in driving community awareness and uptake of ACDs.The Digital Pack, co-designed with the community, includes workshop materials, a digital resource kit, YouTube videos, and volunteer training materials. A key feature is the new ‘You CAN Ask That’ video series, addressing the top ten community questions about ACDs to improve the confidence of peer volunteers in community outreach.The 2019 Lacey Review identified low ACD uptake among older Australians and recommended a stronger role for volunteers and Local Government. In response, strategic funding from the Office for Ageing Well and SA Health supported an ACD collaborative of Local Councils, leading to the development of the Compass Model—a systems-based approach to overcoming barriers and empowering older individuals through peer volunteering. As the model expanded across 13 councils, the Adelaide Hills Council and City of Onkaparinga recognised the need for a single repository for resources and spearheaded the creation of a centralized and accessible Digital Resource Pack.This project is unique in facilitating advance care planning conversations early, within a non-clinical community setting. It has used community codesign to inform the contents which harnessed the power of peer support to empower individuals as experts in their own lives. The ongoing uptake of the community workshops, peer volunteer roles and ACD completion is a testament to its success.This work is important to share to acknowledge council’s commitment to public health and the potential for further partnerships that can address the evolving needs of an ageing population while maintaining appropriate scope of practice.The model and the resources pack are innovative because of the flexibility of its application and use of online learning software. Different councils received funding for different elements these included the needs of metropolitan areas, regional or rural areas and the needs of communities and individuals with a diverse life experience (CALD, LGBTQI, AATSI). The library of digital resources allows any council to access any element as needed.The presentation aligns with several conference themes by detailing project implementation and clarifying the systemic enablers integral to the project’s success and ongoing uptake. Strategically it is being shared within the community engagement stream because of its focus on community education, codesigned tools and resources, and the collaboration of peer volunteers who have a lived experience.
