Traditional illness‐trajectory models describe three broad patterns: a relatively predictable decline typical of many cancers; a fluctuating course with intermittent exacerbations, often seen in organ failure; and a gradual, prolonged decline commonly associated with frailty and dementia. While these models are not intended to capture individual variation, they have provided a practical framework for communicating prognosis and guiding clinical decision-making. 1
