Disseminated spinal infections are characteristically demonstrated in Mycobacterium tuberculosis infections. However, we present a rare case of an elderly male with a disseminated Staphylococcus aureus infection, developing disseminated spinal infections with spondylodiscitis and right sacroiliitis with an associated right psoas and sacral collection. Moreover, secondary to the infection, the patient subsequently developed a large mycotic pseudoaneurysm of the right internal iliac artery. S. aureus is a virulent pathogen; however, haematogenous dissemination affecting multiple musculoskeletal sites is relatively uncommon. We will illustrate and discuss CT and MRI findings and their limitations in demonstrating the disseminated spinal infections and paraspinal collections and the subsequent development and treatment of the mycotic pseudoaneurysm.
