Pigmented spitz nevus is a rare benign melanocytic lesion that may clinically mimic malignant melanoma, particularly when occurring at unusual sites such as the genitalia.We report a case of a child presenting with congenital phimosis associated with blackish discolouration of the foreskin. Circumcision was performed and histopathological examination of the excised foreskin revealed a pigmented spitz nevus, a rare benign melanocytic lesion. This case highlights the importance of careful evaluation of pigmented lesions of the foreskin and the role of histopathology in establishing a definitive diagnosis.
