Three patients who had undergone cataract surgery in the past presented to our clinic with complaints of gradual decrease in vision. One patient had a local consultation and was referred following a failed attempt at neodymium-doped yttrium aluminium garnet (Nd-YAG) laser posterior capsulotomy—the cause of poor vision was thought to be posterior capsular opacification (PCO). On slit lamp examination, whitish appearance of the intraocular lens (IOL) was noted, and the cause of reduced vision was attributed to diffuse opacification of the IOL ( figure 1A, C and E) in all eyes. Scheimpflug imaging was performed in all eyes which showed hyper-reflectivity at the anterior surface of the IOL (figure 1B, D and F). Figure 2A,B show the appearance of IOL on Scheimpflug imaging in pseudophakic eyes with normal IOL characteristics. All three eyes underwent IOL exchange. The manufacturer details and model of the IOLs were unavailable but all were identified as hydrophilic material with a dual haptic design. Table 1 shows the clinical profile and management outcomes of the three patients.
