This 2020–2024 systematic review analysed 79 cohort studies from 13 countries (63 abstracts and 16 manuscripts) including 13 899 virally suppressed individuals who switched to LA-CAB/RPV. Definitions of virological failure (VF) varied, and 27 cohorts did not define VF. Among 172 VF events, 80 had genotypic data: Non-nucleoside reverse transcriptase inhibitor (NNRTI) resistance mutations appeared in 45, Integrase inhibitor (INI) mutations in 40, and dual-class resistance in 33. Time on LA-CAB/RPV was ≤6 months in 38% of cohorts, 6–12 months in 22%, >12 months in 15% and unreported in 13%. Post-VF regimens were described in 23 studies (92 participants), mostly protease inhibitor-based or oral INI-based; 27% continued LA-CAB/RPV. Resuppression occurred in 88% of VF cases. Standardised VF definitions and longer term data are needed.
