Extranodal extension in head and neck cancer: why HN-CLEAR matters and what still needs proof

The Head and Neck Consensus Language for Ease and Reproducibility (HN-CLEAR) initiative provides international recommendations and harmonised terminology for the histopathological diagnosis of extranodal extension (ENE) in head and neck squamous cell carcinoma (HNSCC).1 ENE is a pivotal postoperative risk feature that influences staging, adjuvant therapy and the interpretation of outcomes across studies; however, the field has long lacked prescriptive microscopic criteria for what should and should not count as ENE in routine practice. A systematic review identified 44 unique histopathologic ENE definitions and noted prevalence ranging roughly 21%–85% across comparable series, which is a perfect ‘why HN-CLEAR exists’ data point.2 As a result, ‘ENE-positive’ cohorts are not necessarily interchangeable across papers or institutions, complicating both prognostic inference and treatment-decision generalisability. HN-CLEAR offers a practical, microscope-level solution to reproducibility, while highlighting a separate need: outcome validation under standardised adjudication.
