Biliary adenofibroma (BAF) is a rare benign tumour of the liver. So far, 42 cases have been reported in the literature, 26 of them showed signs of malignant transformation. Since there are no specific features on imaging, it raises the problem of differential diagnosis with malignant tumours and other benign tumours. Thus, liver biopsy is mandatory, and diagnosis is based on histology. Its potential risk of transformation to cholangiocarcinoma makes surgery the surest treatment for BAF.
