Objectives The 2023 EULAR and 2025 ACR SLE recommendations endorse a treat-to-target (T2T) strategy aiming for sustained remission or low disease activity (LDA), with prednisone (PDN) <=5 mg/day and withdrawal when possible. However, real-world data on T2T attainability, particularly GC-free remission, remain limited. It is unclear whether these targets are aspirational or achievable in routine care. Our goal was to assess the proportion of SLE patients attaining the recommended target of PDN-free remission and other T2T endpoints in an academic lupus clinic.Methods Cross-sectional study including all adult patients fulfilling the 2019 EULAR/ACR and/or the 2012 SLICC classification criteria and under active follow-up at a tertiary lupus clinic, where a T2T strategy is in practice. Clinical and laboratory data were collected from the first outpatient visit in 2025. Disease activity at the index visit was primarily assessed using the SLE Disease Activity Score (SLE-DAS), which was used to define target states according to validated thresholds: remission (clinical SLE-DAS=0 with PDN<=5 mg/day), PDN-free remission (clinical SLE-DAS=0 with PDN=0), LDA (SLE-DAS<=2.48 with PDN<=5 mg/day, including remission), and active disease (failure to meet the remission or LDA definitions). SLEDAI-2K was scored and applied to define remission according to the Doria definition (clinical SLEDAI-2K=0 with PDN<=5 mg/day). We recorded the current glucocorticoid dose (PDN-equivalent in mg/day), use of hydroxychloroquine, conventional synthetic immunosuppressants, and biologic therapies. Continuous variables are presented as mean±SD or median (IQR), and categorical variables as n(%).Results 321 patients were included (88.2% female; mean age 49.8±15.4 years; mean disease duration 17.1±10.4 years). The cumulative prevalence of lupus nephritis since diagnosis was 37.1%. SDI>0 was observed in 42.4% of patients. The primary T2T endpoint of PDN-free remission was achieved by 63.2% of patients. SLE-DAS remission and LDA criteria were fulfilled by 80.7% and 83.8%, respectively. Overall, 70.7% were PDN-free. Active disease was present in 16.2% of patients; among these, 46.2%, 9.6%, and 26.9% had mild, moderate, and severe disease activity, respectively.
