S127 Exploring the effect of different respiratory diseases on patients’ perspective of antimicrobial resistance: an AMR-lung/European lung foundation global patient survey

Introduction Antimicrobial resistance (AMR) is a global health threat for people with chronic respiratory disease (CRD). It is crucial to understand what such individuals understand by AMR and how they feel it affects them.Aim To further understand the burden of AMR from the perspective of people with CRD and identify any differences dependent on primary respiratory diagnosis.Methods An online, multilingual, anonymous patient survey was developed by the AMR-Lung ERS Clinical Research Collaboration and European Lung Foundation patient advisory groups. Multivariable logistic regression models were used to analyse disease-specific burden and factors affecting AMR perception across different respiratory diseases.Results Among 1,801 respondents from 57 countries globally ( figure 1), 31% recalled having an AMR-associated infection. Commonly self-reported diseases included bronchiectasis (40.8%, n=735), Cystic Fibrosis (CF) (27.2%, n=489), asthma (25.3%, n=455), and COPD (14%, n=252). Patients who reported diagnosis of longer history CRD (>5 years) were significantly more likely to report AMR-associated infection (OR=1.37 [95% CI 1.01–1.86, p=0.041], as well as those with genetic respiratory diseases such as CF or primary ciliary dyskinesia (OR=2.77 [95% CI 2.01–3.80, p<0.001). Patients with airways diseases were significantly less likely to report prior knowledge of the concept of AMR (OR=0.68 [95% CI 0.47–0.97, p=0.035). Patients with bronchiectasis were significantly more likely to report a perceived lack of antimicrobial effectiveness (OR=1.67 [95% CI 1.07–2.60], p=0.024), use of antimicrobials without medical prescription (OR=1.59 [95% CI 1.10–2.29], p=0.014), and perceiving AMR as a personal threat (OR=2.68 [95% CI 1.64–4.39], p<0.001).Abstract S127 Figure 1Respondents’ country of residence with colours representing the number of respondents. Created with mapchart.netConclusions This global survey is the first to suggest a high AMR burden and threat from the perspective of people with CRD, with disparities across different diseases highlighting the need for targeted education strategies for different patient groups.
