P156 Prevalence of cardiopulmonary events and risk factors before COPD diagnosis: ABACOS RISKS study

Following COPD diagnosis, individuals are at increased risk of cardiopulmonary events. Little is known about the occurrence of cardiopulmonary events and risk factors before COPD diagnosis.Electronic health records of incident COPD cohort were identified (identification period 2018–2019) from CPRD Aurum linked with HES from England. Core cardiopulmonary events and broad cardiopulmonary risk factors were pre-specified and quantified for cases of incident COPD prior to index date (first observed COPD diagnosis) and among incident COPD with ≥3 years of pre-index data. Age, sex and ethnicity matched individuals without COPD served as controls.Of 77,461 incident COPD cases (median age 68 years, 52% male, 93% White), 76.2% had at least 1 core cardiopulmonary event at any time before COPD diagnosis. Compared to controls, patients with COPD had 3.8 (95% CI: 3.7–3.9) times greater odds of having an antecedent core cardiopulmonary event. Of cases with ≥3 years of pre-index data, 56.6% had a recorded core cardiopulmonary event within the 3 years leading to COPD diagnosis with 4.9 (95% CI: 4.8–5.0) times greater odds.Abstract P156 Table 1Cardiopulmonary events and risk factors are highly prevalent before COPD diagnosis, with over half the individuals having a core cardiopulmonary event in the 3 years before COPD diagnosis. Early recognition and management of cardiopulmonary events could improve early detection of and outcomes in COPD.
