A middle-aged Sri Lankan man reported progressively impaired horizontal gaze to either side for 3 weeks, with normal vertical eye movements. He had no diplopia, dysarthria, dysphagia or limb weakness. Over the same period, he also had nausea, persistent vomiting, loss of appetite and low-grade fever. He had a history of chronic alcohol use.
