Chronic limb-threatening ischaemia (CLTI) is characterised by ischaemic rest pain lasting more than 2 weeks and non-healing wounds. Ischaemic rest pain is the result of a complex pathophysiological process, with nociceptive and neuropathic components. Over time, peripheral and central sensitisation occurs, likely linked to the activation of N-methyl-D-aspartate (NMDA) receptors. 1 Pain worsens and becomes increasingly difficult to control.
