Continuity of care has long defined the identity and clinical effectiveness of family medicine and primary care. Longitudinal relationships allow clinicians to understand patients in context and translate that knowledge into better preventive care, chronic disease management and coordinated care. Yet in many contemporary delivery systems, continuity is increasingly treated as optional: desirable when convenient but sacrificed when operational pressures mount.
