Cases of Ebola virus disease in the eastern Democratic Republic of the Congo are rising, with warnings that this outbreak risks becoming one of the “deadliest on record” (doi:10.1136/bmj-2026-727772).1 On 17 May the World Health Organization (WHO) declared a public health emergency of international concern, the third such declaration due to Ebola since 2005 (doi:10.1136/bmj-2026-313572).2 And yet the world is not only underprepared for another global health crisis but moving further away from the international funding and cooperation needed.
