The superspecialty training programme in gastroenterology and hepatology is intended to transform internal medicine physicians into competent, independent specialists. However, increasing concern is being voiced by programme directors, faculty and academic mentors about the erosion of core clinical competencies among gastroenterology trainees. The issue is not limited to one country or region but is reflected in broader conversations around postgraduate medical education. In the context of low- and middle-income countries, this decline is especially concerning given the dual burden of patient volume and disease complexity in tertiary academic centres.
